Coming home and other transitions while moving through the stages of treatment can be the most trying times as well as the most hopeful. Your teen or young adult has gone through a valuable experience and hopefully, has had a growth experience and learned skills that they can work on within the family unit to sustain the elements of treatment that lead to long term success. These times are all about the Transition. Coming back to people, places, and things that may trigger their subconscious to fall back into old patterns. During these times it can be helpful to have an accountability coach independent from either the former treatment facility or familiar home, a blank relationship that can develop as the transition takes place and support it from a place of neutrality. Change occurs when a person makes the decision to act differently over and over again. Having independent support towards those decisions can guide your child and the family through this transitional stage.
Getting your child into the right program is only half the work. The harder part is what happens after: the step-down, the return home, the transition to college, the first months in sober living. These are the moments when structure disappears and the risk of relapse or crisis is highest.
Interactive Youth Transport provides clinically supervised companion and aftercare services for adolescents and young adults navigating exactly these moments. A trained clinical support person placed directly with your child, wherever they are, for as long as they need it.
This is not a placement service. It is a presence. A real relationship, built around your child's specific situation, with measurable goals and clinical oversight behind every step.
What Companion and Aftercare Support Looks Like
A companion is not a babysitter, a case manager filing paperwork, or a staff member clocking in and out of a facility. They are a trusted clinical presence embedded in your child's daily life.

Your child is returning home after residential treatment, wilderness therapy, or a therapeutic boarding school and the transition feels fragile

A note on clinical fit: Not every situation calls for a companion. We will tell you honestly if a different level of care is more appropriate. Our clinical team reviews every inquiry and will not place a companion where the need does not match.
The Clinical Foundation
IYT and Coast Health Consulting are part of the same clinical family under Interactive Health LLC. That means companion placements are not a separate, disconnected service bolted onto the end of transport. They are a continuation of the same clinical oversight, the same standards, and the same commitment to individualized care.
What sets this apart
Masters-level clinical oversight. Every companion placement is supervised by licensed clinicians, not unlicensed contractors. The same standard that defines IYT's transport model applies here.
No referral fees from programs. We do not accept commissions or fees from treatment programs, sober livings, or any placement. When we recommend a setting or a next step, it is because it is right for the client.
Coordinated, not siloed. Companions work alongside existing outpatient providers, treatment teams, and educational consultants. We do not replace your existing professional network. We fill the gap it cannot cover.
Nationwide coverage. Companions are placed locally, near where your child lives, studies, or is in treatment. We have coverage across all 50 states through our network of vetted clinical professionals.
