Intervention, Transport & Clinical Case Management for Bay Area Families

Transport represents a single day. The treatment trajectory that follows — placement monitoring, clinical progress evaluation, step-down planning, provider coordination, and reintegration — requires sustained case management that most families cannot coordinate independently.

Coast Health Consulting, IYT's clinical case management division, provides ongoing coordination for Bay Area families throughout the treatment process. The case manager maintains direct communication with the treatment team, reviews clinical documentation, monitors progress against measurable benchmarks, and prepares for the transitions that follow, step-downs, home visits, program changes, and discharge. Beyond that the Case Manager is the support that is a constant resource throughout all stages of treatment. Too often does a person go to inpatient, then complete the program and go to outpatient, and have to develop new therapeutic relationships from scratch with every level of care. Our Case Managers are the links in the chain to see the youth as a whole and when momentum is found, grab onto it and motivate them towards their goals, and long lasting success and happiness.

For Bay Area families, case management also involves building continuity between the residential program and the local providers who will support reintegration — psychiatrists, family therapists, and academic support teams in the San Francisco metro area. These handoffs between treatment levels are where outcomes are most frequently compromised, and structured case management exists specifically to prevent gaps in clinical continuity.

Above all else Case Management is a third party whose priority is the clients holistic wellbeing. While we work with many professionals and programs, we do so on a merit basis. We do not accept referral fees from treatment programs. Placement recommendations are based on clinical fit — the match between the client's specific diagnostic profile, treatment history, and needs and the program's demonstrated capabilities. This independence is particularly relevant in a treatment landscape where financial incentives frequently influence referral patterns.

Therapeutic Coaching & Companion Services

Not every clinical situation requires transport or intervention. Therapeutic coaching and companion services provide sustained, skilled support for young people who are home but struggling — post-treatment reintegration, stalled transitions to independence, or ongoing behavioral health challenges that benefit from consistent professional involvement outside the family system.

Coaching engagements place a trained behavioral health professional in the client's daily life on a regular basis. The focus is practical: accountability structures, therapeutic skill implementation, routine building, and real-time support through the situations that test recovery — social reentry, academic demands, and the emotional triggers that persist after treatment ends.

Companion services extend to travel support for young people who need professional accompaniment to and from programs, appointments, school transitions, or family events that carry clinical risk. These engagements are planned and managed as clinical interventions, staffed by professionals with behavioral health training.
